Process
Welcome to Dr. Rob McIntosh Dental. Our intake process requires you to complete a couple of forms.
These forms include our "New Patient Information" form and our "Medical History Form". Both will help us get to know you better and help expedite our administrative process prior to your arrival.
These forms can be filled out by downloading and emailing them to us directly. Simply, click on the form to the right, and we'll follow up with you once we've received them.
